This patient illustrates the classic secondary gynecomastia treatment that resulted from the previous surgeon simply not removing the tissue under the nipple-areola complexes so as to theoretically avoid a depression or contour deformity. This is outdated thinking. The tissue in question happens to be the most critical tissue (hormonally sensitive tissue starting right under the nipple-areola complexes). If it is not removed it will be easily felt by the patient, it will continue to make for “puffy nipples” and the risk for recurrence is more than if it were totally removed (published in my paper: Total Removal of Gynecomastia Tissue.

This result is subtle but very real (just ask the patient). Also not that there is an improved contour of his chest with enhanced muscle definition by treatment of the entire chest. His enhanced skin color after his revision is a testament to his lack of fear of taking off his shirt! Secondary gynecomastia is far more challenging that any “never been operated upon patient” and requires skill and experience. Never settle. Never compromise. Seek out the most qualified surgeons wherever they are!
