TGynecomastia in those who have lost a significant amount of weight is a different beast because of the associated skin laxity that requires careful evaluation/consideration. Will the skin retract well with my standard treatment or do I need to do skin removal with scars across the chest and around the areolas? That is the million dollar question. In most cases I can confidently predict how well the patient will do without skin removal but not 100 percent. I can always remove the breast like appearance and with a shirt the patient enjoys a normal appearance but if the goal is to look good in your “speedo” (bare chested) you will need to work your result carefully to get your skin flat and smooth without creases or folds. Many patients don’t have any problems with chest scars while others find this unappealing. I’m all about making sure that you completely understand your options and that your expectations are as realistic as possible. In the end, I want a happy patient who has confidence. This patient flew in from the coast thinking he had some fluid. This was not the case at all (he is two months out). I really believe that he wanted to show off his new chest to me. He is happy. He is grateful. No doubt he is liberated from his gynecomastia burden. Just another day doing my thing at the AGC!
