This is an advanced gynecomastia case for your education. He has a history of significant weight loss (over 60 pounds) and this resulted in sagging chest tissue and skin laxity. I consider this case advanced because a decision needs to be made regarding the type of treatment that is best for his chest anatomy and his goals. I’ll bet many surgeons believe that he needs skin removal with scars across the chest and around his areolas. My experience, based on well over 5000 cases at the Austin Gynecomastia Center is that his skin is likely to retract reasonably well and I can avoid surgical scars across his chest. How do I know this?—been there many times before and I have observed that is all cases the skin retracts to some degree and the nipple-areola complexes move upwards and inwards (I’m working on a paper that proves this). It’s a judgment call, and it’s based on clinical experience which is only obtained after treating many patients and assessing their results.
He is seen here 6 weeks out from his standard gynecomastia treatment with removal of all gross gynecomastia tissue. Skin retraction is very good (although not perfect with fine creases). His nipple-areola complexes are smaller and better located and there is no sagging skin/tissue present. He is early in his recovery so I know he needs to work his result for weeks to months so it’s even better than it is now.
This is what I call a very good result considering what we started with—it is not perfect but it is perfectly amazing and he is very happy and no doubt this has been a life changing experience for him.
