Patient Details

This case illustrates the importance of managing your recovery. Among many things that I have learned at the Austin Gynecomastia Center is that the patient recovery period is very important to the success of my gynecomastia treatment. I often tell my patients they should not worry about what happens at the AGC with my treatment, they need to worry about how they are going to manage their recovery. Invest in your recovery and you give yourself the best opportunity for a normal, flat chest with even, smooth skin.

My critical paper published in 2022 clearly showed that complete gynecomastia tissue removal is not only possible but critical to the superior results that my patients expect. In addition, the number one reason for a post treatment revision was because of scar tissue and not a depression deformity. Scar tissue is a normal consequence of surgery but in some cases, it needs to be actively controlled so it doesn’t negatively affect your result.

The issue of skin elasticity has a big impact on your recovery as well. Those who have poor quality skin that doesn’t have much of an ability to snap back after stretched are at risk of skin folds and creases that will compromise your result. The most at-risk patients are those who are big cases—large female looking breasts with sagging and stretch marks. Many have a significant weight loss history, and many are simply overweight. These are patients whom I discuss the issue of skin removal or no skin removal treatment alternatives.

In this patient’s case, he is 4 months out from treatment and has a crease on his right side. You can actually see this crease in his post operative day 1 photo (see arrows). There is no doubt that if he had worked his skin as I would have, he likely wouldn’t have this crease. The problem is the average patient doesn’t always understand the details and degree of having to work the result so this will not happen in the first place. This is why I am constantly providing educational materials for the benefit of my patients.

What I did for this patient is I injected some steroids in the area of scar adherence on his right chest and instructed him to do deep tissue massage using the roller, his hands or even a mechanical massage gun. With time alone this will get better. This often takes over a year. Patience is a virtue and when I tell you to wait it’s because these issues get better with time. I have seen many patients who I thought needed a crease revision only to find it went away several months later when they were finally scheduled for treatment.
