Patient Details

This case offers much to learn from. This patient had lost a lot of weight and this resulted in well localized sagging of his chest tissue with enlarged areolas. Based on his appearance it would be easy to determine that he needs skin removal to make the areolas smaller and to tighten the skin. My experience with cases like his is that there is the distinct possibility that by removing the dense, heavy tissue behind his areolas his skin will retract well, although likely to be imperfect.

His photos reveal remarkable retraction of his skin and shrinking of the areolas. There is no skin excess. I had to work with him during the recovery period to deal with some scar tissue. He also had a seroma that developed that required aspiration.

What we can all learn:

  1. Skin retraction after gynecomastia treatment can be impressive. Seek out the advice of an experienced gynecomastia surgeon with regards to the need for skin removal or non skin removal treatment.
  2. Scar formation is not uncommon after gynecomastia treatment. Managing the scar tissue that develops and being patient during the recovery period will often reward you. Time is on your side.
  3. Managing your recovery is essential to the best result. Avoid seromas/hematomas and manage scar tissue. If your skin is not flat after treatment you need to work it flat.
  4. All areolas will shrink after tissue removal. The need to make areolas smaller is often not necessary and the associated scarring can be quite poor.
  5. The appearance of this patient’s chest after mass removal and skin retraction depends solely on his underlying anatomy.
  6. For my patients at the AGC, we always want perfect results but completely accept perfectly good results because that’s just the way it is. In his case, I can place his nipple-areola complex into a more “ideal” position but he is perfectly fine the way it is now. He owns his new chest.
